PossAbiliites’ Day Services gives adults with learning and physical disabilities the opportunity to meet new people, learn new skills and take part in meaningful activities which are outcome focussed.

We create stimulating and engaging activities for people with a range of different needs and abilities. Activities may be at one of our four Wellbeing Hubs in Rochdale Borough or in our three bases in BRADFORD, and include:

We've opened exciting new opportunities in Bradford, these are:

The Learning Zone:

Based in the heart of a busy community and near to Bradford college and university, The Learning Zone provides an environment for people to access a wide range of learning opportunities, particularly beneficial to individuals who really wish to continue learning new skills and develop new areas of interest, but for whom an accredited college learning environment might not currently suit.

The Learning Zone provides a variety of courses, to support people with a learning disability to grow and personally develop.

Courses are taught by qualified external tutors and supported by PossAbilities staff, one to one if needed is also available to ensure everyone can participate and benefit. The Learning Zone covers a variety of subjects responsive to individual’s interests and knowledge.  We also work with local colleges and Workers Education Association who deliver training.

Supported employment and volunteering: Professional support and activities for those looking to transition to the world of work or volunteering.

E4 - A safe place to develop life skills

E4 Offers opportunities for people with learning disabilities to access their local community. We work in partnership with local services giving people the chance to be a real part of their community, joining community groups and supporting local events. This enables people to try new things whilst doing something worthwhile and contributing to local services.

The hub offers a safe place for people to have a voice, build on their life skills, develop confidence and socialise with their peers.

Springfield Garden

Springfield Gardens is a horticultural/conservation project for adults with a learning disability. Set on a 7acre site on the edge of Holmewood with orchards and growing plots. The people we support are involved in growing seasonal fruit and vegetables and selling them to the local community. We also provide handicraft/woodwork opportunities.  The people we support are also involved in looking after the animals we have on site.

Who is it for?

Anyone with a learning or physical disability most people who come to our day centres live in Rochdale, Heywood, Middleton or Manchester, but you can come from anywhere.

How to access the service

Just get in touch. The details are all on this page. A social worker, care manager or health professional can refer you, though that’s not essential.  There is a charge per session though this can be met from a persons personal budget if they have one.

Supported Living & Outreach Services

The Community Support Team provides flexible person centred services to young people and adults with learning disabilities with varying support needs including personal care.

We currently provide support services in a number of properties across Greater Manchester, Merseyside and West Yorkshire. Support packages are provided after initial assessment and are tailored to individual needs to develop the skills necessary to improve and maintain independence.

We provide just the right amount of housing related support to enable the people we support to manage their own tenancy within their local community .

Shared Lives Service

Shared Lives is where a person with a spare room, is paid to support a vulnerable adult in their own home, so that they become just like a family member. It’s a bit like fostering – but for grown ups. People who provide placements and support under Shared Lives are referred 
to as Shared Live Carers. This service offers both long term and short term placements. It can be used as day support, as home from hospital care and as a stepping stone for someone to get their own tenancy. Individuals supported in Shared Lives arrangements may have learning or physical disabilities, they may be older people who are frail or living with dementia or mental health problems. Could you help?

We need Shared Live Carers of all ages and genders and ethnicities to make a positive difference to someone’s life. As a Shared Live Carer your role would be to assist someone to live as independently as possible, to improve their self-esteem and confidence and to develop their life skills such as looking after themselves, domestic skills, preparing meals, budgeting and making friends. You could help if:
  • You have a spare room
  • Able to share your home with an adult
or young person
  • Have the time to support an individual to live independently and be part of their local community

Types of Placements

We need people that can offer short terms placements, from one night to several weeks, as well as people who can provide long term support which can be for a year and longer. We also require people who can provide day care support within their home or the community which may range from a few hours each week to longer periods as required. The different groups of people who need placements include:
  • Adults with disabilities
  • Adults with mental health needs
  • Older people
  • Vulnerable young adults

Homeshare

When one gives a spare room, and another offers up to 10 hours of social support in return…That’s a Homeshare!

Often people just need a little bit of support – shopping, gardening, cooking, whilst others, who could provide help, find it hard to get onto the property ladder or save for their tuition fees.

Homeshare brings them together. It’s a win-win!

Respite, Breaks and Supported Holidays

Services Offered

The aim of the service is to provide respite for parents and carers and a pleasurable break from home for the individuals that they care for. We actively promote independence, choice, dignity, privacy and safety. We respect the values, culture rights and beliefs of the diverse group of people whom we support. We cater for all needs and will design a service around individual needs. A menu of respite opportunities including overnight respite, outreach services, short break holidays, drop in events, day trips and support within the home are available. We have a dedicated and professionally trained staff team who care and are sensitive to service user needs and are available throughout the day to provide support in lots of different ways. Who is it for?

Anyone with a learning or physical disability or other needs who is a resident of Rochdale, Heywood, Middleton or Manchester. Possabilities Respite services are registered with CQC, (Care Quality Commission) they inspect services to ensure they are Safe, Effective, Caring, Responsive and Well-led. How to Access the Service

Referrals to our Respite Services come from a variety of health care professionals including social workers, district nurses or care managers. Families and individuals can access the service direct by contacting Harelands House. Once a referral is received, a manager from PossAbilities Respite Services will arrange an initial visit to the house for the person, usually with whoever referred them, and their family member / carer.

Harelands House

Harelands House is a five bedroom, two storey detached house providing 24 hour supported respite services. The house has been fully adapted to meet the diverse needs of the service user group and has a communal lounge and dining room. The outside of the house is fitted with ramps, and two downstairs disabled access bedrooms are complete with training hoist, electric hydraulic beds and wet rooms with adapted facilities to promote independence, including a closemat toilet, grab rails and adapted bathroom facilities.

Leighton House

Our newest respite property is Leighton House, situated near to the picturesque Hollingworth Lake . The house is a modern and vibrant , with contemporary décor , and comprises of 5 bedrooms , each fitted with a television and a DVD player and en suite facilities . The house has two communal lounges and a dining room and has been fully adapted to meet the diverse needs of the service user group and has 2 communal lounges and dining room. The outside of the house is fitted with ramps, and two downstairs disabled access bedrooms are complete with training hoist, electric hydraulic beds and wet rooms with adapted facilities to promote independence, including a closemat toilet, grab rails and adapted bathroom facilities. Service Users are offered a range of activities within the local community and at the nearby Hollingworth Lake , and in house have internet access and the use of games consoles.

Supported Holidays

We realise just how important holidays are for everyone. That’s why we have our own holiday lodge at the  amazing Ribby Hall Viallage, just outside Blackpool. The prices are heavily discounted for the people we support and we can build a package of  support around the person  so that they can really make the most of their time away.

PossAbilities Employment Team

employer_small       Greater Manchester Working Well Programme (GMWW) In partnership with Pure Innovations we deliver the GMWW programme across Rochdale, Bury, Oldham, Tameside and Stockport. This programme is designed to get people with learning disabilities and mental health diagnoses into real paid jobs. Workchoice and Specialist Employability Support (SES)

PossAbilities Employment also delivers the Workchoice Programme and SES throughout the Borough of Rochdale. Workchoice is a DWP funded programme, which PossAbilities Employment is sub contracted to deliver in partnership with Shaw Trust and the European Social fund.

SES is also a DWP funded programme, aimed at those furthest from the job market, which we are subcontracted to deliver in partnership with Shaw Trust, 3SC, the Royal National College for the Blind and the European Social Fund.

At PossAbilities we are all too aware of the difference having a job can make to someone’s life. It can help build self- esteem confidence and lead to more independence. Over the years the service has supported hundreds of individuals to secure employment. We work with service users to identify individual skills and strengths and then match these with a prospective employer. We have established relationships with a number of employers and local colleges. Over the last 25 years we have supported a range of people to access employment, volunteer and or training opportunities.

Who is it for?

Residents from Rochdale, Heywood, Middleton and Manchester aged between 18-65 with a learning disability, a diagnosed mental health condition and or physical or sensory impairment. We also support people with autistic spectrum disorders, Asperger’s syndrome, attention deficit disorder and or severe dyslexia.

Dementia Day Care

Woodclough Day Centre

Woodclough offers stimulation and support to older people with a Mental Health Condition, Alzheimer’s, Dementia or Social Isolation. The Service aims to maintain people’s dignity and rights and also help promote independence. We offer a range of daily activities and the Centre is warm and welcoming and is specially equipped for older people. For more information click here.

Services Offered

We provide a comprehensive programme of stimulating and engaging activities for older people who find it difficult to cope at home because of illness, dementia, a physical disability or a mental health need. Our activities cater for a range of different needs and include:

  • Current affairs discussion group
  • Cards
  • Memory games
  • Musical bingo
  • Music therapy
  • Exercise
  • Relaxation room
  • Help and advice with transport
  • Rehabilitation

Payment

We have to charge for the day care service we offer. The charge is based on your individual circumstances. A person with Dementia can get their care paid for by the Local Authority. Typically this is four visits a day from a carer- morning, lunch, dinner and bedtime. Often these visits can be as short as 15 minutes and can cost about £360 per week. By cutting out the lunch and dinner visits, the same person could spend a morning or afternoon in good company at Woodclough Day Centre. We do interesting things, give help with medication , even give a drink and a sandwich to take home. For a little more than a price of a couple of cups of coffee life can have some meaning again . If you think your relative has Dementia ask your GP to give a diagnosis. If Dementia is diagnosed the Local Authority will fund a package of support for them.There is also a fixed charge for lunch.

Anyone who is a resident of Rochdale, Heywood, Middleton or Manchester. We also provide support and advice to those who may be caring for an elderly relative, friend or neighbour.
